At Foundation House, we provide a structured reintegration model through our residential program, designed to prepare clients for a successful transition to life post-treatment. Each stage of our program is meticulously designed to address the unique challenges and needs of our clients, ensuring they receive the optimal level of care at each phase of their recovery journey.

We understand that successful treatment is far from a one-size-fits-all solution. It’s a deeply personal journey that involves the individual, their family, and preparation for the world that awaits them post-treatment. Our goal is simple: to equip the men we serve with the tools they need for an exciting and fulfilling life firmly rooted in long-term recovery.

Stage 1: Partial Hospitalization Program (PHP)

The journey at Foundation House begins with our Partial Hospitalization Program (PHP) for substance abuse and mental health treatment, following a client’s stabilization and detox. During this initial stage, we focus intensely on individual needs within a structured environment to minimize distractions and enhance stability. Our comprehensive clinical approach includes educating clients about addiction, substance abuse, and mental health, providing robust support through behavioral health and mental health services, and integrating families into the treatment process.

Clients engage in 20+ hours of clinical care weekly, including daily group therapy, individual therapy sessions, and experiential activities such as wilderness adventures, life skills training, wellness activities, and local excursions. Our medical director meets with each client weekly for medical oversight and medication management as needed. We use a creative and flexible approach to treatment, tapping into the innate strengths of our clients while employing evidence-based clinical practices.

At Foundation House, we understand that one-size-fits-all solutions don’t work for everyone. That’s why we customize our program to meet each individual’s unique needs. This personalized approach fosters true success, acknowledging that success is defined uniquely by each client.

Unlike many programs, our clients don’t just attend sessions for 20 hours a week. Our comprehensive program extends through the day, night, and weekends, with varied therapeutic activities more effective than traditional classroom settings, ensuring continuous engagement and growth.

Stage 2: Intensive Outpatient Program (IOP)

Progressing to Stage 2, the Intensive Outpatient Program (IOP) focuses on fostering independence while continuing the momentum of clinical and behavioral support. Clients participate in more than 9 hours of clinical care each week, including one to two individual therapy sessions, clinical assignments, and daily group sessions. Additionally, clients meet with our medical director bimonthly and engage in comprehensive case management.

The structure of Stage 2 still includes daily process groups, support meetings, and experiential activities that promote the development of a robust social support system. Activities include wilderness expeditions, educational support, wellness retreats, and international travel opportunities. Family engagement continues with therapy sessions, regular check-ins, and bi-weekly support group calls, culminating in bi-annual, on-site family workshops in Portland that combine therapeutic work with experiential learning and family-oriented activities. Stage 3: Outpatient Program (OP)

The final stage, our Outpatient Program (OP), prioritizes personal relationships, environmental integration, and goal setting to ensure a seamless transition to life after treatment. This stage integrates academic and vocational pursuits with recovery, allowing clients to continue their education or career development with ongoing support. Clinical care is tailored to align with each client’s long-term goals, facilitating a personalized recovery process.

Clients collaborate with our clinical team to design a daily and weekly schedule that best suits their recovery needs. Continuing care plans involve detailed discussions with clients, families, and clinicians to establish a clear, long-term strategy for maintaining recovery. Even after transitioning out of our treatment program setting, clients remain connected to Foundation House through participation in athletic leagues, alumni groups, international travel, and sponsorship opportunities. Our vibrant alumni program plays a crucial role in supporting the successful transition to independent living.
